Purpose of the Study:

  • To assess psychosocial determinants as explanatory variables for work disability duration.
  • To estimate the predictive value of specific psychosocial factors from the Quickscan questionnaire for sick leave length.
  • To compare the predictive power of psychosocial factors against biomedical determinants, such as diagnosis.

Main Methods:

  • A cohort study of 4,981 insured Belgian patients was conducted.
  • Sick leave duration was analyzed using Kaplan-Meier survival analysis.
  • Cox regression and concordance index (C-index) were employed to identify predictive psychosocial determinants and compare model performances.

  • Fourteen psychosocial determinants, including health perception, workload, and job satisfaction, were significantly related to sick leave length (p<0.10).
  • A biopsychosocial model incorporating these determinants achieved a high predictive capacity (C-index: 0.80).
  • In a subset analysis, the psychosocial model demonstrated superior predictive value (C-index: 0.73) compared to the biomedical model (C-index: 0.63) for predicting long-term sick leave.

Conclusions:

  • A set of 14 psychosocial determinants effectively predicts the length of sick leave.
  • Psychosocial factors offer better predictive capacity for long-term sick leave than diagnostic information alone.
  • These findings highlight the importance of considering psychosocial aspects in managing work disability.
