Background:

Purpose of the Study:

  • To compare clinical outcomes of immediately loaded conventional versus mini-implants for mandibular overdentures.
  • To evaluate marginal bone loss and periodontal health indicators.

Main Methods:

  • Fifty edentulous patients received either conventional (4.1mm) or mini-implants (≤2.9mm) for mandibular overdentures.
  • Implants were immediately loaded with locator attachments.
  • Marginal bone loss, probing depths, plaque, and bleeding indices were assessed over 12 months.

Main Results:

  • Mini-implants showed significantly higher bone loss and changes in plaque and bleeding indices at 12 months compared to conventional implants.
  • Peak insertion torque also differed significantly between the groups.
  • Locator height was identified as a risk factor for bone loss.

Conclusions:

  • Two mini-implants are more susceptible to bone loss following immediate loading for mandibular overdentures.
  • The height of the locator attachment may be a significant risk factor for bone loss in these cases.
