CPAP re-initiation for sleep apnea shows lower adherence than initial trials. Women in the re-CPAP group had significantly less adherence than men, highlighting a need for further research.
Area of Science:
- Sleep Medicine
- Respiratory Therapy
Background:
- Many patients do not succeed in their initial Continuous Positive Airway Pressure (CPAP) therapy for sleep apnea.
- Limited data exists on the effectiveness of re-initiating CPAP therapy for patients who previously failed treatment.
Purpose of the Study:
- To compare the success rates of CPAP re-initiation versus initial CPAP therapy for sleep apnea.
- To investigate factors influencing CPAP adherence, including gender.
Main Methods:
- A retrospective study included 224 patients for CPAP re-initiation (re-CPAP) and 228 CPAP-naïve patients.
- Data collected included patient characteristics, sleep study results, and CPAP therapy outcomes after at least one year.
Main Results:
- The re-CPAP group had more severe sleep apnea compared to the CPAP-naïve group.
- One-year CPAP adherence was 52% for the re-CPAP group, significantly lower than 67% for the control group (P = .001).
- Women in the re-CPAP group showed significantly lower adherence than men (P = .002), unlike the control group where no gender difference was observed.
Conclusions:
- Re-initiating CPAP therapy for sleep apnea results in significantly reduced adherence compared to initial trials.
- CPAP acceptance after re-initiation is lower in women than in men.
- Further research is needed to understand the gender disparity in CPAP re-initiation success.
