Background:

Gallbladder stones are highly prevalent, with 3-10% of patients experiencing concomitant common bile duct stones (CBDS).
Preoperative diagnosis of gallbladder lithiasis has advanced with imaging technologies like ultrasound and magnetic resonance cholangiography.

Observation:

A novel technique utilizing single-operator cholangioscopy (pSOC) under laparoscopic guidance was employed for a patient with large bile duct stones.
The procedure involved choledocotomy and primary bile duct closure, with the patient tolerating it well.

Findings:

The combination of laparoscopy and pSOC is a feasible, safe, and effective treatment for CBDS.
The new pSOC device (SpyGlass Discover) offers improved maneuverability compared to standard cholangioscopes.
