Left ventricle non-compaction (LVNC) is a rare cardiomyopathy. Early diagnosis and management of heart failure are crucial, as heart transplantation is the only definitive treatment for this condition.

Background:
- Left ventricle non-compaction (LVNC) is a rare congenital cardiomyopathy.
- It is characterized by abnormal myocardial development, leading to a spongy appearance of the left ventricle.
- LVNC can result in severe heart failure, arrhythmias, and embolic events.
Observation:
- A 40-year-old male presented with cardiogenic shock and multiorgan failure.
- Echocardiogram showed severe four-chamber enlargement and LVEF <10%.
- CMR revealed hypertrabeculation, myocardial thinning, and a left ventricular thrombus, consistent with LVNC.
Findings:
- The patient experienced refractory heart failure and required inotropic support.
- Despite treatment, he suffered ventricular tachycardia arrest and succumbed.
- A high index of suspicion is necessary for early LVNC diagnosis.
Implications:
- Management focuses on heart failure, arrhythmias, and embolic events.
- No specific therapy exists for LVNC.
- Heart transplantation is the only definitive treatment for advanced LVNC.
