Summary

Primary lymphocytic hypophysitis, an autoimmune pituitary disease, often affects postpartum women. Early steroid treatment in a postpartum patient rapidly resolved visual defects and headaches, normalizing pituitary size.

Background:

  • Primary lymphocytic hypophysitis (PLH) is a rare autoimmune disorder affecting the pituitary gland.
  • PLH commonly presents during pregnancy or the postpartum period, causing pituitary destruction and mass effects.
  • Symptoms include headaches, visual disturbances, ophthalmoplegia, and hypopituitarism.

Observation:

  • A 38-year-old postpartum woman experienced decreased visual acuity and headache.
  • MRI revealed a sellar mass with concurrent low free thyroxine and cortisol levels.
  • Clinical presentation suggested PLH.

Findings:

  • Treatment with high-dose prednisone (60 mg/day) and hormone replacement therapy was initiated.
  • Within one month, pituitary size decreased, and visual field defects improved.
  • Long-term steroid therapy (36 months) with gradual tapering led to a normal pituitary gland size at two-year follow-up.

Implications:

  • Early diagnosis and prompt immunosuppressive therapy are crucial for managing PLH.
  • Steroid treatment can effectively reverse pituitary inflammation and mass effect in PLH.
  • This case highlights the importance of considering PLH in postpartum women with pituitary abnormalities.
