Erectile dysfunction (ED) in germ cell tumor (GCT) survivors improves over time, independent of age or treatment. Early follow-up shows higher rates of ED, which significantly decreases after two years.

  • Germ cell tumors (GCTs) are common cancers in young adults, with high cure rates.
  • Maintaining survivors' quality of life (QOL) is crucial post-treatment.
  • Erectile dysfunction (ED) negatively impacts QOL in GCT survivors.

Purpose of the Study:

  • To investigate the relationship between ED and patient/treatment factors in GCT survivors.
  • To identify predictors of ED in this population.

Main Methods:

  • Assessed ED using the Sexual Health Inventory for Men (SHIM) questionnaire.
  • Measured serum hormone levels in survivors.
  • Analyzed relationships between SHIM scores, age, hormone levels, follow-up duration, and treatment using logistic regression.

Main Results:

  • Of 52 survivors, 46 completed the SHIM; median age was 38, follow-up 35 months, SHIM score 18.
  • 85% had SHIM scores <22, indicating potential ED; 46% had scores <17.
  • ED rates were 69% within 2 years of follow-up, improving to 33% after 2 years.
  • Longer follow-up was the only independent factor associated with improved SHIM scores.

Conclusions:

  • SHIM scores improve post-GCT treatment, suggesting ED resolution over time.
  • Improvement is independent of patient age, hormone levels, or specific treatment modalities.
  • Focusing on extended follow-up may aid in managing ED in GCT survivors.
