  • Angiosarcoma is a rare, aggressive cancer with high recurrence rates after standard treatment.

Purpose of the Study:

  • To systematically review the literature on neoadjuvant systemic therapy for angiosarcoma.
  • To evaluate the impact of neoadjuvant therapy on surgical resection success.
  • To assess the effect of neoadjuvant therapy on long-term patient survival.

Main Methods:

  • Systematic literature review of studies published before October 2019.
  • Databases searched include Ovid Medline, Embase, Cochrane Library, and Scopus.
  • Included were 18 case reports and 6 retrospective cohort studies; no randomized controlled trials were identified.

Main Results:

  • Neoadjuvant chemotherapy demonstrated a beneficial effect in reducing tumor size (downstaging).
  • Improved resection margins were observed, particularly in cardiac and cutaneous angiosarcoma.
  • Evidence regarding a definitive impact on long-term survival is lacking.

Conclusions:

  • Neoadjuvant chemotherapy may facilitate less extensive surgeries and increase the rate of negative resection margins.
  • Further prospective studies, potentially through an international registry, are needed to establish survival benefits.
  • Consideration of neoadjuvant chemotherapy is advised for angiosarcoma management.
