Synthesis and Regulation of Thyroid Hormones01:20

Synthesis and Regulation of Thyroid Hormones

6.7K
Low blood levels of the thyroid hormones — triiodothyronine (T3) and thyroxine (T4) — signal the hypothalamus to release the thyrotropin-releasing hormone (TRH). TRH then reaches the pituitary gland and stimulates the release of thyroid-stimulating hormone(TSH) into the bloodstream.
Upon reaching the thyroid gland, TSH stimulates the follicular cells' active uptake of iodide ions from the blood. The ions diffuse to the apical surface of the cells and are oxidized to iodine. The...

The Thyroid Gland01:23

The Thyroid Gland

6.3K
The thyroid gland is a small, butterfly-shaped gland located in the neck and covers the anterior surface of the trachea. The gland has two lateral lobes connected by a thin tissue mass called the isthmus. Internally, each lobe comprises many small spherical structures known as thyroid follicles, surrounded by a network of blood vessels.
The follicles have a central cavity lined by simple cuboidal to squamous epithelial cells called follicular cells. These cells produce the glycoprotein...

Hypothalamic-Pituitary Axis01:37

Hypothalamic-Pituitary Axis

64.8K
The response to stress—be it physical or psychological, acute or chronic—involves activation of the Hypothalamic-Pituitary-Adrenal (HPA) axis. The HPA axis is part of the neuroendocrine system because it involves both neuronal and hormonal communication. Its function is to regulate homeostatic systems—metabolic, cardiovascular, and immune—providing the necessary means to respond to a stressor.

Functions of Thyroid Hormones01:18

Functions of Thyroid Hormones

4.6K
The thyroid hormone (TH) plays a pivotal role in the intricate orchestration of physiological processes, exerting profound effects on development, metabolism, and homeostasis throughout different life stages.
TH is indispensable for the normal development and maturation of the skeletal, muscular, and nervous systems during fetal and childhood growth. It facilitates bone mineral turnover and regulates protein synthesis in developing tissues, contributing significantly to overall growth and...

Area of Science:

  • Endocrinology
  • Computational Biology
  • Medical Diagnostics

Background:

  • The hypothalamus-pituitary-thyroid axis has a unique homeostatic set point for each individual.
  • This set point oscillates narrowly within broader population ranges of free thyroxine (FT4) and thyroid-stimulating hormone (TSH).
  • Thyroid function tests (TFTs) assess these circulating hormones, but individual variations can be significant.

Purpose of the Study:

  • To develop and evaluate a mathematical algorithm, Thyroid-SPOT, for reconstructing an individual's unique euthyroid set point.
  • To assess the accuracy of Thyroid-SPOT in predicting set points using post-thyroidectomy patient data.
  • To determine the clinical utility of Thyroid-SPOT in guiding personalized thyroid hormone replacement therapy.

Main Methods:

  • Developed the Thyroid-SPOT mathematical algorithm to reconstruct personalized set points.
  • Applied Thyroid-SPOT to serial TFT data from 101 patients post-total thyroidectomy.
  • Compared predicted set points against pre-operative euthyroid TFTs using Bland-Altman analysis.

Main Results:

  • Bland-Altman analysis showed small mean differences: +3.03 pmol/L for FT4 and -0.03 mIU/L for TSH.
  • These differences are minimal compared to the wide reference intervals of population TFTs.
  • Thyroid-SPOT demonstrated high accuracy in predicting the euthyroid set point, particularly for TSH.

Conclusions:

  • Each individual possesses a unique equilibrium euthyroid set point.
  • Thyroid-SPOT is an accurate and reliable system for personalized euthyroidism restoration.
  • The algorithm can assist clinicians in L-thyroxine titration for patients with persistent symptoms despite 'normal' TFTs.
