  • A middle-aged metalworker presented with unilateral nasal obstruction due to a mass in the right middle meatus.
  • Surgical excision revealed a granulomatous lesion, posing a diagnostic dilemma.

Findings:

  • Clinicopathological correlation and expert consultation led to a diagnosis of a pseudotumor.
  • The lesion was linked to occupational exposure to water-soluble cutting oils, termed "Suds oil".

Implications:

  • This report describes the first known association between "Suds oil" inhalation and sinonasal pseudotumor formation.
  • It expands the spectrum of occupational sinonasal diseases and underscores the importance of considering workplace exposures in differential diagnoses.
