Radiologists should recognize rare primary liver cancers beyond hepatocellular carcinoma (HCC) and intrahepatic cholangiocarcinoma (ICC). Accurate diagnosis of these uncommon hepatic malignancies is crucial for patient prognosis and treatment.

Background:

Hepatocellular carcinoma (HCC) and intrahepatic cholangiocarcinoma (ICC) are the most common primary liver malignancies.
Radiologists must also be aware of less common primary hepatic tumors.
Accurate diagnosis impacts patient treatment and prognosis.

To review rare primary hepatic malignancies encountered in clinical practice.
To highlight cases initially misdiagnosed as HCC or ICC.
To discuss the radiographic and pathologic features, prognosis, and treatment of these rare tumors.
