Local Anesthetics: Clinical Application as Intravenous Regional Anesthesia

933
Intravenous regional anesthesia or the Bier block technique is used to anesthetize a specific limb or extremity. It uses exsanguinated or blood-drained vessels to transport local anesthetics or LAs to the peripheral nerve trunks. Lidocaine without vasoconstrictors like epinephrine is most commonly used for this technique. Other drugs used are prilocaine, ropivacaine, and chloroprocaine. Bupivacaine is not recommended for this technique due to its high cardiac toxicity.

Cardiopulmonary Resuscitation II: ACLS Airway Management

353
Airway management is a key skill in emergency and critical care settings, as maintaining a clear airway is essential for adequate oxygenation and ventilation.Head Tilt-Chin Lift TechniqueThe head tilt-chin lift maneuver is an essential technique primarily used in patients without suspected cervical spine injuries. To perform this maneuver, one hand is placed on the patient’s forehead, and gentle pressure is applied backward to tilt the head. The fingertips of the other hand are positioned...

    Summary

The OMNA Marine Tourniquet is effective for self-application, including one-handed use in marine environments. It achieves high pressures, comparable to other tourniquets, ensuring effective occlusion.

    Area of Science:

    • Medical Devices
    • Emergency Medicine
    • Trauma Care

    Background:

    • The OMNA Marine Tourniquet is a 5.1cm-wide device designed for marine environments.
    • It features a simple redirect buckle, hoop-and-loop security, and a ratcheting mechanism.
    • This study aimed to evaluate its self-application effectiveness and achieved pressures.

    Purpose of the Study:

    • To assess the effectiveness of the OMNA Marine Tourniquet during self-application.
    • To measure the pressures generated during application on limbs.
    • To compare its performance characteristics with existing tourniquets.

    Main Methods:

    • Sixty subjects performed thigh and arm self-applications.
    • Secured, occlusion, and completion pressures were measured in triplicate.

  • Applications included pulling down/up on thighs and single-handed arm applications (circumference ≥30cm).
  • Main Results:

    • All 540 applications were effective.
    • Secured pressures exceeded 150mmHg in 376 of 453 applications.
    • Occlusion pressures averaged 348mmHg (thighs) and 285mmHg (arms); completion pressures averaged 414mmHg (thighs) and 344mmHg (arms).

    Conclusions:

    • The OMNA Marine Tourniquet demonstrates effective self-application, including one-handed use.
    • Achieved occlusion and completion pressures are comparable to the 3.8cm-wide Ratcheting Medical Tourniquet.
    • The device is suitable for use in marine environments.
