Background:

Dual mobility cups (DMCs) are increasingly used in primary total hip arthroplasty (THA), accounting for 15% of cases in Denmark in 2018.
Intraprosthetic dislocation (IPD) is a known but rare complication associated with THA, particularly with DMC use.

Observation:

An 83-year-old male patient experienced atraumatic hip dislocation five weeks after primary THA with a DMC.
Closed reduction under sedation in the emergency department resulted in IPD.

Findings:

The case demonstrates that closed reduction of a dislocated THA with DMC can lead to IPD.
Specific repositioning protocols are crucial for managing dislocations in THA with DMC.

Implications:

Repositioning a dislocated THA with DMC should be performed under general anesthesia with muscle relaxation and fluoroscopic guidance.
This approach aims to minimize the risk of IPD and ensure optimal patient outcomes following THA.
