Background:

  • Bone-conduction hearing devices (BCHD) offer excellent outcomes for pediatric hearing loss.
  • Pediatric BCHD implantation presents unique surgical challenges due to developing bone and atypical anatomy.
  • Image-guided surgical navigation (IGSN) is underutilized in BCHD placement despite its potential benefits.

Observation:

  • IGSN was employed in three pediatric cases with complex otologic anatomy.
  • Cases included syndromic patients with variable temporal bone thickness and patients with prior mastoidectomies.
  • All patients underwent successful BCHD implantation without significant intra- or post-surgical complications.

Findings:

  • IGSN facilitated BCHD (BAHA and BONEBRIDGE™) implantation in pediatric patients with complex temporal bone anatomy.
  • The technology helped identify optimal surgical sites, minimizing risks.
  • All three patients achieved favorable audiologic outcomes post-surgery.

Implications:

  • Intraoperative IGSN is a valuable tool for pediatric BCHD placement, especially in complex cases.
  • IGSN can enhance surgical precision and reduce morbidity associated with BCHD implantation.
  • This study expands the application of IGSN to BAHA and BONEBRIDGE™ device placement in challenging pediatric otologic surgeries.
