  • Secukinumab is an interleukin-17A inhibitor used for psoriasis, psoriatic arthritis, and ankylosing spondylitis.
  • Paradoxical reactions, including cutaneous vasculitis, are rare but documented side effects of secukinumab.
  • Only four cases of secukinumab-induced vasculitis were previously reported globally.

Purpose of the Study:

  • To report the first case of cutaneous vasculitis induced by secukinumab in Brazil.
  • To raise clinical awareness regarding this rare adverse effect of secukinumab.

Main Methods:

  • Case report of a patient with peripheral spondyloarthritis treated with secukinumab.
  • Clinical observation of adverse drug reaction (cutaneous vasculitis).
  • Management involved drug withdrawal and initiation of etanercept.

Main Results:

  • The patient developed cutaneous vasculitis during secukinumab treatment.
  • Discontinuation of secukinumab and initiation of etanercept led to good resolution of the vasculitis.
