Summary

Prophylactic mesh closure effectively reduces incisional hernia after colorectal surgery. While mesh closure increased surgical site infections, it significantly lowered hernia rates in this study.

Background:

  • Incisional hernias are a common complication following laparotomy for colorectal surgery.
  • Limited research exists on prophylactic mesh closure to prevent these hernias.

Purpose of the Study:

  • To evaluate the efficacy of prophylactic mesh closure in preventing incisional hernias after open colorectal surgery.

Main Methods:

  • Retrospective cohort study comparing patients with and without prophylactic mesh closure.
  • Included 309 patients undergoing open colorectal surgery between January 2014 and December 2016.
  • Follow-up averaged 21.7 months.

Main Results:

  • Prophylactic mesh closure was associated with a significantly lower incidence of incisional hernia (9.2% vs. 25.7%, p=0.001).
  • Multivariate analysis indicated a protective effect of mesh closure (Hazard Ratio = 0.46, p=0.033).
  • Surgical site infections were more frequent in the mesh group (19.4% vs. 9.5%, p=0.015).

Conclusions:

  • Prophylactic mesh closure is an effective strategy to decrease incisional hernia incidence after colorectal surgery.
  • The benefit of hernia prevention may outweigh the increased risk of surgical site infection in selected patients.
