Summary

Laparoscopic nephrectomy for giant hydronephrotic kidneys is challenging. A new "Pneumorenum" technique uses suction and CO2 inflation for easier dissection and improved surgical outcomes.

Background:

Giant hydronephrosis presents surgical challenges, particularly during laparoscopic nephrectomy.
Large, nonfunctioning kidneys occupying significant abdominal space complicate dissection and increase operative risk.

Observation:

Laparoscopic dissection of giant hydronephrotic kidneys is technically demanding due to their size and distension.
Standard laparoscopic approaches may be hindered by the sheer volume of the pathological kidney.

Findings:

The
Pneumorenum
technique involves initial decompression via suction through a 5 mm trocar under direct vision.
Subsequent controlled inflation with carbon dioxide gas moderates kidney size, facilitating easier laparoscopic dissection.

Implications:

This technique simplifies the laparoscopic approach to giant hydronephrotic kidneys.
The
Pneumorenum
method offers a potential solution for improving safety and efficiency in complex laparoscopic nephrectomies.
