Background:

Purpose of the Study:

  • To investigate the impact of common comorbidities on processing speed test (PST) performance in MS patients.
  • To analyze the relationship between comorbidities, cognitive function (PST), and quantitative MRI metrics over time.

Main Methods:

  • Retrospective, longitudinal analysis of 4,344 MS patients with PST data (June 2015 - August 2019).
  • Comorbidities assessed included diabetes mellitus (DM), hypertension (HTN), hyperlipidemia (HLD), and depression.
  • Quantitative MRI metrics (WBF, T2LV, GMF, TV) were analyzed alongside PST scores using linear regression models.

Main Results:

  • Over half of MS patients had at least one comorbidity; depression was most common (37.4%).
  • Comorbidities were associated with lower baseline PST scores; two comorbidities led to a decline of 1.46 PST points/year.
  • Depression correlated with lower PST scores and worse MRI outcomes (lower WBF/GMF, higher T2LV). HLD showed favorable MRI metrics but mixed PST results.

Conclusions:

  • Comorbidities adversely affect processing speed in MS patients, with depression showing significant negative impacts on cognition and brain structure.
  • Hyperlipidemia presented a complex association, with favorable MRI but less consistent cognitive effects.
  • The findings highlight a complex interplay between comorbidities, cognition, and neuroimaging in MS, suggesting that baseline processing speed influences sensitivity to comorbid conditions.
