Guidelines for Nursing Documentation I01:30

Guidelines for Nursing Documentation I

1.5K
Quality documentation and reporting share essential characteristics that ensure they are practical and valuable resources for those who use them. These characteristics are:
Factual:
The following points emphasize the significance of upholding accurate and unbiased documentation in healthcare.

Dosage Interval and Administration Route: Determination Methods

94
A medication’s effectiveness largely depends on its appropriate dosage and the route of administration. Dosage ensures that a sufficient drug concentration is maintained in the bloodstream to elicit the desired therapeutic effect without causing toxicity. The route of administration affects the drug's bioavailability, rate of absorption, and onset of action, which are crucial for achieving optimal therapeutic outcomes. Drug dosage calculations are critical to tailoring therapy to...

Dosage Regimens: Designs and Approaches

131
Designing a dosage regimen, which refers to the manner of drug administration, is a complex process involving the selection of drug dose, route, and frequency. This process is underpinned by pharmacokinetic parameters derived from tests and population averages. These parameters are then tailored to patient-specific variables such as diagnosis, demographics, and allergy status. Once therapy commences, therapeutic response monitoring is critical and achieved through clinical and physical...

Drug Therapy01:28

Drug Therapy

158
The advent of drug therapy has profoundly shaped modern mental health care, providing targeted treatments for a range of psychological disorders. Psychotherapeutic drugs, classified into antianxiety, antidepressant, and antipsychotic medications, address symptoms across anxiety disorders, mood disorders, and schizophrenia. While these medications have transformed patient outcomes, they require careful management due to their potential side effects and limitations.

Errors occurring during blood pressure monitoring01:25

Errors occurring during blood pressure monitoring

1.2K
Blood pressure monitoring is a crucial clinical procedure in diagnosing and managing various cardiovascular conditions. Despite its significance, the accuracy of blood pressure measurements can be compromised by multiple factors, potentially leading to either falsely high or low readings. These inaccuracies are critical as they can significantly impact patient care. So, it is vital to understand these challenges deeply and adopt strategic approaches to minimize errors.
Several factors...

Drug Dosing: Geriatric Patients

111
Elderly individuals encompass a diverse population with varying degrees of age-related physiological changes. Defining the elderly presents challenges, as the geriatric population is often arbitrarily categorized as individuals older than 65. However, many individuals in this group lead active and healthy lives, with an increasing number surpassing 85 years and falling into the older elderly category. Physiological changes associated with aging impact performance capacity and homeostatic...

Area of Science:

  • Nursing Education
  • Patient Safety
  • Pharmacology

Background:

  • Post-discharge adverse drug events are a significant national concern.
  • Effective inpatient medication instruction is crucial for patient safety.
  • Persons with cognitive challenges (PWCCs) often require specialized teaching methods.

Purpose of the Study:

  • To evaluate the impact of errorless teaching/learning methods plus pictorial medication cards (ETL + card) on registered nurse (RN) teaching.
  • To assess the effect of ETL + card on medication adherence among PWCCs.
  • To determine if this intervention improves medication self-management in PWCCs.

Main Methods:

  • A pre-post intervention study design was employed.
  • Data were collected from RNs and PWCCs on a rehabilitation unit.
  • Intervention involved RNs using ETL + card for medication instruction.

Main Results:

  • RNs utilized significantly more teaching strategies post-intervention (p = .003).
  • Patient prescription fill rates improved from 67% to 85%.
  • RN teaching satisfaction increased from 0% to 50%.

Conclusions:

  • ETL + card intervention enhanced RN medication teaching.
  • The intervention showed a potential positive impact on patient medication adherence.
  • ETL + card may support safe medication self-management for PWCCs.
