Summary

Dental implants with a partial machined collar (PMC) and conical connection showed a trend toward better outcomes than those with a flat connection and groovy neck design after three years. Marginal bone loss was significantly lower in the PMC group.

Background:

  • The crest module of a two-piece implant is crucial for retaining prosthetic components and maintaining peri-implant tissues.
  • Different implant-abutment connection designs, such as conical connection with partial machined collar (PMC) and flat-to-flat connection with groovy neck, may influence long-term clinical outcomes.

Purpose of the Study:

  • To compare the three-year clinical and radiographic outcomes of prosthetic rehabilitations on conical connection implants with PMC (CC Group) versus same body-designed implants with flat-to-flat connection and groovy neck (FC Group).

Main Methods:

  • A retrospective chart review was conducted on patients with NobelReplace CC PMC or NobelReplace Tapered Groovy implants.
  • Data from patients with at least three years of follow-up after loading were analyzed.
  • Outcomes included implant/prosthesis failures, biological/technical complications, and marginal bone loss.

Main Results:

  • Implant survival rates were 98.7% for the CC group and 100% for the FC group; prosthesis survival rates were also 98.7% and 100%, respectively. These differences were not statistically significant.
  • Mean marginal bone loss after three years was significantly lower in the CC group (0.22 ± 0.06 mm) compared to the FC group (0.62 ± 0.30 mm) (p = 0.003).
  • No statistically significant differences were found in biological or technical complications between the two groups.

Conclusions:

  • Implants with conical connection and partial machined collar demonstrate a trend towards superior outcomes compared to implants with flat connection and groovy collar design.
  • The significantly lower marginal bone loss in the CC group suggests potential advantages for long-term peri-implant tissue health.
