Background:

  • Prostate stereotactic body radiotherapy (SBRT) is an effective treatment for early-stage prostate cancer.
  • The Halcyon Linac is a novel radiotherapy system with O-ring geometry.

Purpose of the Study:

  • To evaluate the feasibility, safety, and efficacy of cone beam CT-guided prostate SBRT using the Halcyon Linac.
  • To compare Halcyon VMAT plans with SBRT-dedicated Truebeam VMAT plans.

Main Methods:

  • Volumetric modulated arc therapy (VMAT) prostate SBRT plans were generated for ten patients using the Halcyon Linac with a 6MV-FFF beam.
  • Acuros-based algorithm and Halcyon couch insert were used for heterogeneity corrections.
  • Plans were generated following RTOG-0938 criteria with two-full arc geometry.

Main Results:

  • Halcyon VMAT plans achieved highly conformal dose distributions to the prostate with reduced dose spillage to organs-at-risk (OARs).
  • Treatment delivery time was significantly reduced to under 10 minutes per fraction.
  • Pretreatment quality assurance results were comparable to Truebeam VMAT plans.

Conclusions:

  • Cone beam CT-guided prostate SBRT on the Halcyon Linac is a fast, safe, and feasible treatment option.
  • The Halcyon Linac improves patient compliance and clinic workflow due to shorter treatment times.
  • The study recommends Halcyon Linac for prostate SBRT to increase access to hypofractionated treatments.
