Background:

Binswanger's disease is a form of subcortical arteriosclerotic encephalopathy.
Depression is a common comorbidity in elderly patients with neurological disorders.

Observation:

Four elderly patients with Binswanger's disease presented with prominent depressive symptoms.
Three patients received antidepressant therapy, showing significant improvements in psychological and physical functioning.
One patient, unable to complete therapy due to illness, improved spontaneously over several months.

Findings:

Depression associated with Binswanger's disease appears to be a reversible condition.
Neurological abnormalities may persist despite the resolution of depressive symptoms.
The depressive syndrome is hypothesized to be an organic affective disorder stemming from ischemic white matter lesions.

Implications:

Antidepressant treatment can be effective in managing depression in Binswanger's disease.
