Stroke during migraine is often unexplained. This study suggests prolonged migraine attacks, not cardiac or arterial issues, may cause these strokes in migraine sufferers.

Area of Science:

  • Neurology
  • Cardiology
  • Vascular Medicine

Background:

  • The cause of ischemic stroke during migraine attacks remains unclear.
  • Migraine is a common neurological disorder often associated with severe headaches.

Purpose of the Study:

  • To investigate the underlying causes of ischemic stroke in patients experiencing stroke during a classic migraine attack.
  • To differentiate stroke mechanisms in migraineurs with stroke during migraine versus those with stroke remote from migraine.

Main Methods:

  • Studied 22 patients with ischemic stroke during classic migraine using CT, Doppler ultrasound, cerebral angiography, and echocardiography.
  • Compared findings with age- and sex-matched controls: migraineurs with stroke remote from migraine and non-migraineurs with stroke.

Related Experiment Videos

Main Results:

  • No cardiac or arterial lesions identified in 91% of patients with stroke during migraine.
  • Significantly fewer lesions found compared to controls (9% and 18%, respectively; p < 0.00001).
  • Mitral valve prolapse, arterial dissection, and vasospasm were not significant causes.

Conclusions:

  • Prolongation of the migrainous process beyond usual limits is a likely explanation for most migraine-associated strokes.
  • Stroke during migraine appears distinct from strokes in non-migraineurs or migraineurs with remote strokes.
