Geriatricians face system and professional barriers in developing perioperative medicine for older patients. However, perceived patient needs and unique geriatric skills create opportunities for integrated, multidisciplinary care models.
Area of Science:
- Geriatric Medicine
- Perioperative Care
- Health Services Research
Background:
- Growing development of perioperative medicine services for elderly surgical patients globally.
- Need to understand geriatrician perspectives on integrated geriatrics perioperative medicine services.
Purpose of the Study:
- To explore geriatricians' views on challenges and opportunities in developing integrated geriatrics perioperative medicine services.
Main Methods:
- Qualitative phenomenological study using semi-structured interviews.
- 38 geriatricians from 81 acute public hospitals in Australia and New Zealand were interviewed.
- Thematic analysis using a framework approach was applied to the data.
Main Results:
- System barriers include funding, service overlap, and competing priorities.
- Healthcare professional barrier: lack of role clarity in perioperative teams.
- Facilitators: unmet patient needs, surgical/anaesthetic support, geriatrician skills.
Conclusions:
- Geriatricians face system and professional challenges but see opportunities for innovative care.
- Strong belief in meeting patient needs with geriatric skills drives multidisciplinary models.
- Further qualitative and implementation science research, including patient perspectives, is needed.
