Diagnosing atopic dermatitis in infancy using established diagnostic criteria: a cohort study

Diagnosing atopic dermatitis (AD) in infants is challenging. This study found that while UKWP and H&R criteria are useful in research, their clinical value for infant AD diagnosis is limited.

- Persistence of eczema across visits significantly increased the odds of an AD diagnosis; however, 34.1% of infants meeting H&R criteria did not report itch.
Conclusions:
- The UK Working Party (UKWP) and Hanifin and Rajka (H&R) criteria, when used together and repeatedly, show utility in clinical research settings for infant atopic dermatitis.
- However, the practical clinical value of these criteria for routine infant AD diagnosis appears limited due to inconsistencies and the lack of a key symptom (itch) in a significant proportion of cases.
