Summary

Nephrogenic diabetes insipidus can cause severe urinary tract dilation and kidney damage without obstruction. Temporary drainage can restore kidney function and prevent long-term damage, avoiding major surgery.

Background:

  • Polyuria from diabetes insipidus and psychogenic polydipsia can lead to urinary tract dilation.
  • Renal failure is uncommon in these conditions, even with significant dilation.

Observation:

  • This case details nephrogenic diabetes insipidus causing nonobstructive hydronephrotic renal damage.
  • Massive urinary tract dilation occurred without mechanical obstruction.

Findings:

  • Temporary suprapubic drainage normalized renal function.
  • Upper urinary tract dilation significantly decreased after drainage.
  • Renal function was preserved for over 10 years.

Implications:

  • Nonobstructive hydronephrosis due to polyuric states can be managed conservatively.
  • Temporary vesical drainage is sufficient for managing renal damage.
  • Extensive surgical intervention is typically unnecessary for this condition.
