Summary

Dihydropyrimidine dehydrogenase (DPD) deficiency can lead to severe toxicity from 5-fluorouracil (5-FU) and capecitabine chemotherapy. Current DPD testing is insufficient for routine screening before 5-FU treatment.

Area of Science:

  • Oncology
  • Pharmacogenomics
  • Clinical Toxicology

Background:

  • 5-fluorouracil (5-FU) and capecitabine are standard cancer chemotherapies.
  • Severe, life-threatening toxicity can occur, often linked to dihydropyrimidine dehydrogenase (DPD) deficiency.
  • Current DPD testing methods do not meet criteria for routine pre-treatment screening.

Purpose of the Study:

  • To review a fatal case of capecitabine toxicity.
  • To examine the literature on DPD deficiency screening in patients receiving 5-FU or capecitabine.
  • To highlight the need for improved DPD deficiency screening protocols.

Main Methods:

  • Case study review of a capecitabine-related fatality.
  • Literature search on DPD deficiency and fluoropyrimidine toxicity.
  • Analysis of current DPD testing guidelines and limitations.

Main Results:

  • DPD deficiency is a significant risk factor for severe fluoropyrimidine toxicity.
  • A case of fatal capecitabine toxicity in a patient with presumed DPD deficiency was detailed.
  • Existing DPD screening tests are not universally implemented or validated for routine use.

Conclusions:

  • DPD deficiency poses a critical risk in 5-FU and capecitabine therapy.
  • Current screening practices for DPD deficiency are inadequate.
  • Enhanced DPD screening strategies are essential to prevent severe chemotherapy-induced toxicity.
