Summary

Patients with unilateral stroke often have general visual attention deficits. Right hemisphere stroke patients may also have subtle contralesional attention issues, missed by traditional tests.

Background:

  • Unilateral stroke frequently leads to hemispatial neglect or visuoattentive deficits.
  • While contralesional deficits are common, non-lateralized attention issues are also reported in stroke patients.

Purpose of the Study:

  • To compare spatially lateralized (contralesional) and non-lateralized (general) visuoattentive deficits.
  • To investigate these deficits in patients with left hemisphere (LH) and right hemisphere (RH) stroke.

Main Methods:

  • 40 chronic unilateral stroke patients (20 LH, 20 RH) and 20 controls participated.
  • Contralesional deficits assessed using Bells Test and Lateralized Targets Computer Task.
  • Non-lateralized deficits evaluated with a novel large-screen Ball Rain task.

Main Results:

  • No contralesional deficits found with the Bells Test.
  • RH patients showed more left-sided target misses on the Lateralized Targets Computer Task compared to controls and LH patients.
  • Both RH and LH patients exhibited non-lateralized attention deficits on the Ball Rain task.

Conclusions:

  • Computer-based assessments effectively detect visuoattentive deficits in unilateral stroke.
  • Both LH and RH stroke patients demonstrate non-lateralized visual inattention.
  • RH stroke may involve subtle contralesional deficits missed by traditional cancellation tasks.
