Background:

  • Diffuse Large B-cell Lymphoma (DLBCL) is the most prevalent form of non-Hodgkin's Lymphoma (NHL).
  • Extranodal metastasis is a known characteristic of DLBCL, though adrenal gland involvement is exceptionally uncommon.

Observation:

  • A 65-year-old male presented with symptoms including asthenia, anorexia, hypersudoresis, and significant weight loss.
  • Thoracoabdominopelvic CT revealed bilateral adrenal masses with heterogeneous contrast enhancement, indicative of suprarenal insufficiency.

Findings:

  • Diagnostic workup excluded infectious etiologies and functioning tumors.
  • The patient was ultimately diagnosed with Diffuse Large B-cell Lymphoma (DLBCL).

Implications:

  • This case underscores the importance of considering rare presentations of DLBCL, particularly adrenal involvement.
  • Highlights the diagnostic challenges and the need for comprehensive evaluation in cases of unexplained adrenal masses and systemic symptoms.
