Area of Science:

  • Reproductive Medicine
  • Surgical Gynecology
  • Diagnostic Imaging

Background:

  • Abdominal ectopic pregnancy is a rare condition associated with high morbidity due to atypical presentations and delayed diagnosis.
  • This report details a unique case of abdominal ectopic pregnancy where the placenta invaded the rectal wall.

Observation:

  • A 32-year-old woman presented with rising serum human chorionic gonadotropin (hCG) levels after a negative diagnostic laparoscopy for ectopic pregnancy.
  • Imaging revealed an adnexal mass adherent to the rectal wall, raising suspicion for rectal ectopic pregnancy.

Findings:

  • Histological examination confirmed a rectal ectopic pregnancy.
  • Laparoscopic surgery successfully removed the products of conception, leading to an uneventful recovery.

Implications:

  • Abdominal pregnancies, particularly rectal ectopic pregnancies, pose significant diagnostic and management challenges.
  • Early diagnosis requires integrating clinical findings, serial hCG monitoring, and advanced imaging (ultrasound, CT, MRI).
  • Laparoscopic management and a multidisciplinary approach involving gynecologists and gastrointestinal surgeons are essential for optimal outcomes.
