Summary

This study found that while COVID-19 can cause neurological issues, seizures and status epilepticus were secondary to other medical conditions, not direct symptoms of the virus. Most patients showed nonspecific encephalopathy on EEG.

Area of Science:

  • Neurology
  • Infectious Diseases
  • Neurophysiology

Background:

  • Coronavirus disease 2019 (COVID-19), caused by SARS-CoV2, primarily affects the respiratory system but exhibits neurotropic features.
  • Neurological manifestations of COVID-19 include cerebrovascular disease, myopathy, meningitis, encephalitis, demyelination, and seizures.
  • Understanding the specific electroencephalographic (EEG) patterns associated with COVID-19 is crucial for patient management.

Purpose of the Study:

  • To analyze continuous video-electroencephalography (EEG) findings in patients with COVID-19.
  • To identify specific EEG features or epileptogenicity related to SARS-CoV2 infection.

Main Methods:

  • Review of continuous video-EEG tracings from SARS-CoV2-positive patients over a two-week period.
  • Analysis of patient demographics, clinical characteristics, imaging, and EEG features.

Main Results:

  • Sixteen COVID-19 positive patients were included in the analysis.
  • Continuous video-EEG was primarily ordered for altered mental status (11 patients) and clinical seizures (5 patients).
  • Seizures and status epilepticus were not presenting symptoms but resulted from COVID-19-related complications like anoxic brain injury, stroke, lithium toxicity, hypertension, and renal disease.

Conclusions:

  • Most COVID-19 patients monitored with continuous video-EEG exhibited nonspecific encephalopathy.
  • Clinical seizures and electrographic status epilepticus were observed as secondary neurological complications in this patient cohort.
