Summary

Early diagnosis of stroke versus transient ischemic attacks (TIAs) is possible. Symptom duration and initial severity reliably differentiate between stroke and TIAs within hours of onset.

Area of Science:

  • Neurology
  • Cerebrovascular Medicine
  • Diagnostic Imaging

Background:

  • Distinguishing between stroke and transient ischemic attacks (TIAs) is crucial for timely treatment.
  • Traditional diagnostic timelines may delay critical interventions.
  • Early differentiation aids in appropriate patient management and resource allocation.

Purpose of the Study:

  • To determine if stroke versus TIAs can be diagnosed earlier than 24 hours.
  • To evaluate the utility of initial neurologic deficit and symptom persistence for early diagnosis.
  • To refine diagnostic criteria for acute cerebrovascular events.

Main Methods:

  • Evaluation of 78 patients with first-time cerebrovascular episodes of presumed ischemic origin.
  • Assessment of neurologic deficit severity at onset and admission.
  • Monitoring of symptom duration and recovery time within the first 24 hours.

Main Results:

  • 50% of TIAs resolved within 1 hour; 90% resolved within 4 hours.
  • Neurologic deficit scores were significantly lower in TIA patients compared to stroke patients.
  • Symptom persistence beyond a few hours was a reliable indicator.

Conclusions:

  • Early diagnosis of stroke versus TIAs is feasible within hours.
  • Symptom duration and initial deficit severity are key diagnostic indicators.
  • This approach can expedite treatment decisions for cerebrovascular episodes.
