Retroperitoneal fibrosis is an uncommon cause of ureteral obstruction in children. This case highlights its potential presentation mimicking other masses in pediatric patients.

Background:

  • Ureteral obstruction in children often stems from congenital anomalies or tumors.
  • Retroperitoneal fibrosis (RPF) is infrequently diagnosed in pediatric populations.

Observation:

  • A 12-year-old boy presented with flank pain due to left ureteral obstruction.
  • Computed tomography revealed an enhancing retroperitoneal mass.

Findings:

  • Pathologic examination confirmed the mass to be retroperitoneal fibrosis.
  • This rare condition mimicked a tumor in its presentation.

Implications:

  • RPF should be considered in the differential diagnosis of pediatric ureteral obstruction.
  • Early recognition can prevent delayed diagnosis and management of RPF.
