Summary

Texture analysis of baseline MRI scans can identify differences in chronic active and chronic stable multiple sclerosis (MS) lesions. While texture features show higher heterogeneity in active lesions, accuracy in differentiating them remains fair.

Background:

  • Chronic active lesions in multiple sclerosis (MS) are linked to clinical disability.
  • Assessing dynamic lesion changes necessitates longitudinal MRI data.
  • Texture analysis (TA) offers a potential method for characterizing MS lesions.

Purpose of the Study:

  • To evaluate the efficacy of texture analysis on baseline T1-weighted 3D MPRAGE MRI scans.
  • To differentiate chronic active MS lesions from chronic stable MS lesions using TA.
  • To explore texture features for identifying lesion activity in MS.

Main Methods:

  • Utilized Voxel-Guided Morphometry (VGM) on 3T MRI scans from 60 MS patients (baseline and 12-month follow-up).
  • Performed texture analysis on baseline 3D MPRAGE images, extracting 36 texture features per lesion.
  • Analyzed 374 chronic MS lesions (155 active, 219 stable).

Main Results:

  • Significant differences in texture features (e.g., GLCM_Energy, GLCM_Contrast) were observed between chronic active and stable lesions.
  • Higher values for specific texture features were noted in chronic active MS lesions.
  • Partial least squares regression achieved an AUC of 0.7 for differentiating lesion types.

Conclusions:

  • Texture features from MPRAGE images suggest increased intralesional heterogeneity in chronic active MS lesions.
  • The accuracy of texture analysis in differentiating chronic active from chronic stable MS lesions is currently fair.
  • Further research may refine TA methods for improved MS lesion characterization.
