A sitting-to-supine fall in vital capacity (ΔVC) of ≤-15% effectively identifies bilateral diaphragm dysfunction (DD). However, ΔVC is not reliable for diagnosing unilateral DD, necessitating careful consideration in clinical evaluations.

- ΔVC is not a reliable predictor for unilateral diaphragm dysfunction.
- Clinical interpretation of ΔVC for suspected DD should consider these findings, particularly differentiating unilateral from bilateral cases.
