Background:

Chronic eosinophilic pneumonia (CEP) is a rare idiopathic interstitial lung disease.
It predominantly affects females and is characterized by eosinophilia.

Observation:

A 27-year-old male with uncontrolled asthma presented with cough, fever, and dyspnea.
Initial diagnosis of pneumonia was unresponsive to antibiotics.
Imaging revealed upper and middle lobe consolidations and air space opacities.

Findings:

Peripheral blood showed 30% eosinophilia.
Bronchoalveolar lavage (BAL) confirmed eosinophil infiltration.
Transbronchial biopsies revealed eosinophil accumulation in lung tissues.

Implications:

This case underscores the importance of considering CEP in patients with unexplained pulmonary infiltrates and eosinophilia.
Early diagnosis and prompt corticosteroid therapy are key for favorable outcomes in CEP.
