Summary

The American Board of Orthodontics (ABO) objective grading system did not correlate with smile esthetics in Class I extraction or non-extraction cases. Incorporating soft tissue variables may improve smile evaluation.

Background:

  • The American Board of Orthodontics (ABO) developed an objective grading system to standardize orthodontic treatment outcomes.
  • Evaluating smile esthetics is crucial in orthodontic treatment planning and assessment.

Purpose of the Study:

  • To assess the correlation between the ABO objective grading system components and perceived smile esthetics.
  • To determine if the ABO grading system can predict smile attractiveness in Class I extraction versus non-extraction cases.

Main Methods:

  • 40 Class I orthodontic patients (20 extraction, 20 non-extraction) aged 13-30 years were analyzed.
  • Post-treatment dental casts and panoramic radiographs were scored using the ABO grading system.
  • Smile attractiveness was rated by a panel of 12 orthodontists; statistical analyses (Pearson correlation, logistic regression) were performed.

Main Results:

  • A weak correlation was observed between ABO grading criteria and smile attractiveness (r values from -0.63 to 0.37).
  • Neither individual ABO scores nor the total score significantly predicted smile attractiveness in either extraction or non-extraction groups (p>0.05).

Conclusions:

  • The current ABO objective grading system does not correlate with post-treatment smile esthetics in Class I cases.
  • The study suggests incorporating soft tissue variables into the ABO grading system for a more comprehensive smile evaluation.
