Background:

  • Infertility affects nearly 14% of US women, incurring significant healthcare costs and psychosocial distress.
  • Tubal factor infertility (TFI), often stemming from sexually transmitted diseases and pelvic inflammatory disease, is a major cause of infertility.
  • Racial disparities in TFI and in vitro fertilization (IVF) prevalence require thorough investigation.

Purpose of the Study:

  • To evaluate and describe potential racial disparities in tubal factor infertility (TFI) prevalence.
  • To assess racial differences in in vitro fertilization (IVF) initiation among women with TFI.

Main Methods:

  • A retrospective cohort study reviewed records of 660 infertile women aged 19-42 from two US infertility clinics.
  • Prevalence of TFI and IVF initiation was calculated overall and stratified by race.
  • Prevalence ratios (PRs) with 95% confidence intervals (CIs) were used to compare groups.

Main Results:

  • Tubal factor infertility (TFI) was diagnosed in 16.7% of women, with a 2-fold higher prevalence in Black women (30.3%) compared to White women (13.9%).
  • While similar proportions of Black and White women with TFI were offered IVF, fewer Black women initiated IVF (6.7%) compared to White women (31.0%).

Conclusions:

  • Tubal factor infertility (TFI) prevalence is significantly higher among Black women compared to White women.
  • Evidence suggests Black women with TFI are less likely to initiate in vitro fertilization (IVF).
  • Enhanced prevention and treatment of sexually transmitted diseases may help reduce TFI disparities.
