Summary

Recurrent abdominal wall endometriosis can occur after surgical repair of an incisional hernia. Early detection and wide local excision are crucial for managing this condition in women with a history of uterine surgery.

Background:

  • Abdominal wall endometriosis is a rare extrapelvic manifestation of endometriosis, occurring in 0.3-1% of cases.
  • This case involves a 48-year-old female with a history of multiple abdominal surgeries, including endometrioma removal and incisional hernia repairs with different mesh types.

Observation:

  • The patient presented with cellulitis and imaging revealed a large infraumbilical mass.
  • Ultrasound-guided puncture and cytology confirmed recurrent endometriosis.

Findings:

  • Recurrence of endometriosis in the abdominal wall was diagnosed.
  • Typical symptoms include cyclical pain and an abdominal mass associated with menstruation.

Implications:

  • Wide local excision with at least 1 cm negative margins is the recommended treatment.
  • High clinical suspicion is warranted in reproductive-aged women presenting with cyclical abdominal pain, a palpable mass, and a history of uterine-related surgery.
