Background:

  • Coronavirus disease-2019 (COVID-19) is a global pandemic.
  • Ophthalmic manifestations of COVID-19 are increasingly reported.
  • Acute outer retinopathy is a rare cause of vision loss.

Observation:

  • A 40-year-old female presented with acute vision loss in her right eye.
  • She had recent symptoms of fever and myalgias and tested positive for COVID-19.
  • Systemic laboratory tests were unremarkable.

Findings:

  • Multimodal retinal imaging revealed findings consistent with white-dot-like outer retinopathy.
  • The patient was treated with systemic prednisone.
  • Significant improvement in vision, symptoms, and retinal imaging was observed within 10 days of steroid initiation.

Implications:

  • COVID-19 can be associated with inflammatory outer retinal disease.
  • This case highlights a rare but significant cause of vision loss in COVID-19 patients.
  • Prompt diagnosis and treatment with corticosteroids may lead to visual recovery.
