Background:

Purpose of the Study:

  • To examine the incidence and predictors of persistent moderate to severe axial musculoskeletal pain (MSAP) and widespread pain six weeks after a MVC.
  • To identify risk factors for persistent pain in Black adults following MVCs.
  • To inform the development of targeted interventions for this population.

Main Methods:

  • Prospective, multi-center cohort study of Black adults presenting to emergency departments (EDs) after MVCs.
  • Data collection included ED assessments and six-week follow-up via surveys or interviews.
  • Multivariable models analyzed predictors of MSAP persistence.

Main Results:

  • High incidence of MSAP (79.8%) and widespread pain (28.3%) was observed in the ED post-MVC.
  • At six weeks, persistent MSAP affected 67% and widespread pain 31% of participants.
  • Predictors of six-week MSAP included older age, peritraumatic dissociation, ED pain severity, uncertainty about recovery, and depressive symptoms.

Conclusions:

  • Black adults presenting to EDs after MVCs face a significant risk of persistent and widespread musculoskeletal pain.
  • Early identification of risk factors like peritraumatic dissociation and depression is key.
  • Preventive interventions are urgently needed to improve outcomes for this high-risk group.
