Background:

  • Effective leadership and teamwork are vital for multidisciplinary intensive care unit (ICU) emergency response.
  • Pulmonary and critical care trainees receive inconsistent training in these essential skills.
  • A standardized leadership curriculum for critical care training is currently lacking.

Purpose of the Study:

  • To develop and implement a longitudinal crisis leadership curriculum for first-year pulmonary and critical care fellows.
  • To enhance leadership skills and boost trainee confidence in managing life-threatening emergencies.
  • To utilize high-fidelity simulation as a primary training and skill-solidification tool.

Main Methods:

  • Curriculum development based on needs assessment, literature review, and expert opinion.
  • Four curriculum sessions conducted over two academic years (2016-2018).
  • Each session included leadership behavior training, simulation, and post-simulation debriefing.

Main Results:

  • 100% of fellows (N=13) completed all curriculum sessions.
  • Participants reported enhanced understanding of leadership, increased confidence, and better preparedness for crisis leadership.
  • Simulation and debriefing were highly effective learning methods, receiving positive feedback.

Conclusions:

  • Implementing a longitudinal crisis leadership curriculum in early fellowship training is feasible and well-received.
  • The curriculum successfully improved leadership competencies and confidence among pulmonary and critical care fellows.
  • Further research is necessary to identify optimal methods for teaching and evaluating leadership skills in critical care.
