A reversible cause of left ventricular dysfunction: Case report and brief review

Background:
- Diabetic ketoacidosis (DKA) is a serious complication of diabetes.
- Cardiovascular complications are frequently associated with DKA, but direct myocardial dysfunction is less commonly recognized.
Observation:
- A patient presented with DKA, exhibiting elevated cardiac biomarkers (troponin I, natriuretic peptides) and ECG abnormalities.
- Transthoracic echocardiogram revealed severe left ventricular systolic dysfunction, initially diagnosed as profile L heart failure.
Findings:
- Despite initial severe dysfunction, left ventricular function normalized within seven days of DKA treatment.
- Etiologic investigation for other causes of heart failure was negative, strongly implicating DKA.
Implications:
- Metabolic disorders, particularly DKA, should be considered in the differential diagnosis of new-onset left ventricular dysfunction.
- This case highlights a reversible cardiovascular manifestation of DKA, emphasizing the systemic impact of diabetes.
