Language serves as a bridge between ideas and communication, influencing how individuals perceive and interact with the world. Psychologists have long debated whether language shapes thought or vice versa. This discussion gained grip with Edward Sapir and Benjamin Lee Whorf in the 1940s, who proposed that language determines thought, a concept known as linguistic determinism. They suggested that the vocabulary and structure of a language influence how its speakers think and perceive reality.

    Background:

    • Multiple sclerosis (MS) frequently causes language impairments, affecting word retrieval, comprehension, and discourse.
    • These communication challenges negatively impact psychological well-being and interpersonal relationships.
    • Limited research exists on the specific characteristics of language impairment in MS.

    Purpose of the Study:

    • To identify symptom-related, demographic, clinical, social network, and quality of life (QOL) factors associated with language impairment in MS.
    • To explore the multifaceted nature of communication deficits in individuals with MS.
    • To establish a foundation for future research into treatment and causality.

    Main Methods:

    • An international online questionnaire was administered to participants with MS.
    • A forward stepwise regression analysis was employed to assess variable associations.
    • A language impairment index from the Communication and Language Assessment questionnaire for persons with Multiple Sclerosis (CLAMS) served as the dependent variable.

    Main Results:

    • Two hundred and two participants completed the study.
    • The CLAMS language impairment score correlated significantly with self-reported cognitive impairment, speech/voice issues, and QOL.
    • Group membership and participation also showed a significant association with language impairment.

    Conclusions:

    • Self-reported language impairment in MS is significantly associated with symptom, social, and QOL variables.
    • These findings propose an initial model for understanding language impairment in MS.
    • This model can inform future research on therapeutic interventions and causal links.
