Definitive Radiotherapy for Penoscrotal Extramammary Paget's Disease: A Case Report with Long-Term Follow-Up

Background:

• Penoscrotal extramammary Paget's disease (EMPD) is a rare intraepithelial adenocarcinoma.

Observation:

• An 89-year-old male patient with penoscrotal EMPD was treated with radical external radiotherapy (RT).
• The patient received a total dose of 61.8 Gy in 30 fractions using electron and photon beams.
• Treatment tolerance was good with no observed severe toxicity during follow-up.

Findings:

• The patient achieved excellent disease control with no evidence of recurrence at 6.5 years post-RT.
• Definitive RT demonstrated minimal toxicity.
• Radical RT proved effective in managing penoscrotal EMPD in an elderly, inoperable patient.

Implications:

• External radiotherapy is a viable nonsurgical therapeutic option for penoscrotal EMPD.
• RT offers excellent disease control and minimal toxicity, even in extremely elderly patients.
• This approach may significantly improve outcomes for patients with inoperable penoscrotal EMPD.
