Background:
- Penoscrotal extramammary Paget's disease (EMPD) is a rare intraepithelial adenocarcinoma.
- Effective nonsurgical treatment strategies for EMPD are limited.
- This study focuses on an inoperable elderly patient with widespread penoscrotal EMPD.
Observation:
- An 89-year-old male patient with penoscrotal EMPD was treated with radical external radiotherapy (RT).
- The patient received a total dose of 61.8 Gy in 30 fractions using electron and photon beams.
- Treatment tolerance was good with no observed severe toxicity during follow-up.
Findings:
- The patient achieved excellent disease control with no evidence of recurrence at 6.5 years post-RT.
- Definitive RT demonstrated minimal toxicity.
- Radical RT proved effective in managing penoscrotal EMPD in an elderly, inoperable patient.
Implications:
- External radiotherapy is a viable nonsurgical therapeutic option for penoscrotal EMPD.
- RT offers excellent disease control and minimal toxicity, even in extremely elderly patients.
- This approach may significantly improve outcomes for patients with inoperable penoscrotal EMPD.
