Background:

Occult gastrointestinal bleeding can be a sign of underlying pathology.
Appendiceal intussusception is a rare condition, often presenting with non-specific symptoms.

Observation:

A 39-year-old asymptomatic woman presented with positive fecal occult blood test.
Colonoscopy revealed a cecal lesion, confirmed by CT scan as an enhancing focus in the medial cecal wall.

Findings:

Laparoscopic right hemicolectomy was performed after intraoperative findings of an intussuscepted appendix.
Histopathology confirmed appendiceal intussusception secondary to endometriosis.

Implications:

This case highlights the importance of investigating occult bleeding, even in asymptomatic individuals.
Endometriosis should be considered in the differential diagnosis of appendiceal intussusception.
Surgical management is crucial for resolving appendiceal intussusception and its underlying cause.
