Background:

  • Anterior cruciate ligament (ACL) rupture often leads to impaired knee function.
  • ACL reconstruction addresses mechanical instability but may not resolve sensorimotor deficits.

Purpose of the Study:

  • To evaluate sensorimotor function, specifically active joint position sense (JPS), in ACL-reconstructed patients.
  • To compare JPS in ACL-reconstructed individuals against their contralateral limb and healthy controls.

Main Methods:

  • Systematic literature search of multiple databases (MEDLINE, CINAHL, EMBASE, etc.) until April 2020.
  • Included studies focused on active JPS in ACL-reconstructed patients or controls, adhering to PRISMA guidelines.
  • Study quality assessed using a standardized tool; data synthesized descriptively.

Main Results:

  • Ten studies met inclusion criteria.
  • Early post-operative (up to 3 months) showed significant JPS deviation in the affected limb.
  • At 6 months, reconstructed limbs demonstrated significantly less error compared to controls; findings were inconsistent beyond one year.

Conclusions:

  • Sensorimotor function, assessed by JPS, appears altered after ACL reconstruction.
  • The clinical relevance of these sensorimotor changes is questionable due to inconsistent findings and small effect sizes.
  • JPS testing is feasible in acutely injured individuals, and prospective studies are recommended to track sensorimotor recovery during rehabilitation.
