A patient with gallstones experienced severe abdominal pain, leading to the discovery of a liver abscess and psoas muscle inflammation. This case highlights potential complications following gallbladder surgery.

Background:

  • Choledocholithiasis, or gallstones in the common bile duct, can lead to serious complications.
  • Cholecystectomy with choledochotomy and laparoscopic lithoextraction is a standard treatment for choledocholithiasis.

Observation:

  • A 43-year-old female presented with acute abdominal pain three days post-cholecystectomy.
  • Abdominal CT revealed a liver abscess in segment VI, adjacent to metallic density material, and extension of inflammation to the iliac psoas muscle.

Findings:

  • The patient's symptoms were indicative of a possible cholangitis and a complicated liver abscess.
  • The presence of metallic density material near the abscess warrants further investigation for retained surgical items or other causes.
