Summary

Conservative treatment for symptomatic synovial folds in the knee is highly effective, with nearly 90% of patients improving within 60 days. Surgical intervention for the pathological synovial fold is reserved for persistent symptoms.

Area of Science:

  • Orthopedics
  • Sports Medicine
  • Knee Anatomy

Background:

  • The synovial fold is an intra-articular knee structure present in over 50% of individuals.
  • Hypertrophy of the synovial fold, often due to reduced physical activity, can mimic meniscal injury symptoms.
  • Conservative management is typically effective for symptomatic synovial folds.

Purpose of the Study:

  • To present the clinical experience and treatment outcomes for pathological synovial folds of the knee.
  • To evaluate the efficacy of conservative and surgical interventions for synovial fold abnormalities.

Main Methods:

  • A retrospective review of 58 patients (70 knees) treated for pathological synovial folds.
  • 62 knees received conservative treatment; 8 knees underwent surgical intervention.

  • Data collected included patient demographics, treatment modalities, and outcomes.
  • Main Results:

    • Conservative treatment, including activity modification and muscle rebalancing, yielded good results in nearly 90% of patients within 60 days.
    • Surgical arthroscopic resection of the synovial fold, while effective, involves a longer and more challenging rehabilitation.
    • Surgical treatment was indicated for persistent instability, blockage, and pain.

    Conclusions:

    • Non-surgical management of pathological synovial folds is highly successful, achieving positive outcomes in most patients within a defined rehabilitation period.
    • Arthroscopic resection is a viable option for refractory cases, though it necessitates a more extensive recovery process.
    • Early diagnosis and appropriate conservative treatment are crucial for managing synovial fold pathology.
