Celiac Artery Compression Syndrome: A Unique Presentation.

Celiac artery compression syndrome (CACS) is a rare condition causing abdominal pain and weight loss due to median arcuate ligament compression. Surgical decompression offers significant symptom relief for patients.

Background:
- Celiac artery compression syndrome (CACS), also known as median arcuate ligament syndrome, is a rare disorder.
- It results from the median arcuate ligament compressing the celiac artery.
Observation:
- A 44-year-old male presented with 5 years of significant weight loss, abdominal pain, weakness, and lethargy.
- He experienced poor nutritional intake and pain upon eating.
- Physical exam was unremarkable, but mesenteric vascular duplex showed severe celiac trunk stenosis.
Findings:
- Contrast-enhanced computed tomography angiography confirmed CACS with acute angle J-configuration, stenosis, and post-stenotic dilatation.
- CACS can present with post-prandial abdominal pain, weight loss, and an abdominal bruit.
- Diagnostic imaging includes mesenteric vascular duplex, CT angiography, MR angiography, and celiac artery angiography.
Implications:
- CACS is an elusive diagnosis to consider when other causes of abdominal pain and weight loss are excluded.
- Surgical decompression by dividing the median arcuate ligament is the primary treatment.
- Most patients experience significant and long-lasting symptom relief after surgical intervention.
